    A place I used to work at was hogging out a load cell, Rockwell 5150 - type steel, at .100 each pass, when the tool holder AND the workpiece just up and shattered. 3 weeks in the hospital while the wounds and bones healed.
    6 months later when it was his birthday we sent him an advertisement for body armor and said we'd all chip in...

  3. I worked in a machine shop in the 80's and one of the guys showed me a hole up in the ceiling (repaired) and corner where a chuck flew off the lathe and knocked out a hole. Good reminder to always lock the chuck on tight. No one was injured.

    1. Yep, I will bet you saw more than your share of such monkey business during your career... bad things can happen even to the pros when that much metal is moving fast and massive amounts of momentum and mass are involved...
