I had a 180 yr old log cabin farm house with a tiny basement Had a similar near vertical stair that faced a very rough stone wall. I put matching pipe railings on both sides of the steps and never casually climbed up or down. I often wondered how many bodies were buried in the dirt floor
Yup, family has a 100+ year old farmhouse with stairs to the small attic area, pretty much the same setup. No rail, plus the real possibility of impaling your scalp on the many nails protruding thru the ceiling in that tiny closet where the stairs are located. You basically have to crawl up on all fours while keeping your head tucked down.

40 years later, still go down ladders facing out.
